Contents.—The entries in the first column under this heading are descriptive and relative to the symbolical entries in the second column, which latter are as follows: 8. & W.—(Sick and Wounded).—Registers so classed show name, rank, company and regiment, date of admission, diagnosis and disposition. (Note.—A new and enlarged form of Register of Sick and Wounded, together with a Register of Surgical Operations, of similar size, was issued in February, 1864, which is more comprehensive than the earlier Registers— giving eight additional columns for correspondingly fuller statements respecting Patients. They are distinguished on the Index by a cross—viz: “ 8. & W. X.”) 8. & W. and Pres.—(Sick & Wounded and Prescription.)—These Registers are posted daily, and give name, rank, company and regiment, date of admission, disease, prescription and disposition. The disease of the Patient is given at the time of admission only. These Registers are mostly of small size, used in field service. Pres.—(Prescription.)—These books give name of Patient, diagnosis, daily prescription aud disposition. Where the diagnosis is omitted, this fact is stated on the Index in the column of “Remarks.” Wounded.—These entries embrace Surgical Cases, and lists of wounded men only. CASUALTIES.—This term is used in reference to lists of men killed or wounded in action, and in most cases these lists show the character of wounds. Cases.—These books give extended histories of medical or surgical cases, methods of treatment aud general results. N K—31 EXPLANATORY NOTES. Changes.—This term denotes that the entries in the records show admissions, dispositions, furloughs, deaths, discharges, returns to duty and transfers. The disease for which the Patient is admitted will not be found on these records, unless indicated by “ Diagnosis” in “ Remarks” column. O. & L.—(Orders and Letters.)—These books contain originals or copies of Orders, Letters and Endorsements. Cloth.—(Clothing.)—These are records of clothing issued, the baggage and other property of Patients, and inventories of the effects of deceased men. (Arms and equipments are sometimes noted.) Des. Lists.—(Descriptive Lists.)—In these books are noted, usually, the dates of receipt and disposition of the soldier’s Descriptive List. MlSCEL.—(Miscellaneous.) [In “ Class ” column.]—Under this heading are filed books which are numerical or admit of no other classification. Numl. Repts.—(Numerical Reports.)—These, in either daily, weekly or monthly order, give, by numbers, general data of Patients. MONTHLY Reports.—These Reports are not taken up on this Index. They embrace statistical reports from all Regimental and other Hospitals. They also show date and cause of death or discharge, by name (which are entered on the State-alphabetical Death and Discharge Records of this office). They show date and cause of transfer to the Veteran Reserve Corps subsequent to January, 1864.
