It is a list of so- called "tumor terms", i.e., terms which a neophyte indexer would easily recognize as tumors in medical parlance. Since the indexer is advised during his training of the basic familiar tumor words (tumor, cancer, neoplasm, adenoma, carcinoma, sarcoma, nevus, polyp, etc.), he would be expected to know when to look in this tumor manual for indexing instructions on tumors. Such concepts as stromal myosis (ENDOMETRIOSIS) or Paget's disease of breast (CARCINOMA, DUCTAL) he would not easily recognize as neoplastic and would not, therefore, be expected to use this tumor manual as his point of access for instructions on these terms. This manual lists only those terms which are "obviously tumors"; the non-obvious disease terms such as those mentioned above do not appear in the basic alphabetical list of tumors here (they are in Appendix IV instead). On the other hand, there are some "obvious tumor" terms in the list which give indexing instructions to non-neoplastic headings. The terms in this list have been taken from the "Manual of Tumor Nomenclature & Coding" published by the American Cancer Society in 1968. Selected terms have been taken from its predecessor published in 1951. The indexing instructions are those supplied by the Index Section with the cooperation of the Medical Subject Headings Section of the National Library of Medicine for use in INDEX MEDICUS and derivative publications and in the MEDLARS search program. FORM For the sake of brevity and ease of typing, each entry has been kept as simple as possible. A typical entry, squamous cell carcinoma - CARCINOMA, EPIDERMOID is to be interpreted as "squamous cell carcinoma is indexed as CARCINOMA, EPIDERMOID" Indexers and users of indexing tools, especially of MEDICAL SUBJECT HEADINGS (MeSH), are familiar with the inverted form of entry (CARCINOMA, BASAL CELL; TUBERCULOSIS, PULMONARY; SURGERY, OPERATIVE; etc.). All terms listed here, however, will be given in both direct and inverted forms, i.e., both "oat cell car- cinoma" and "carcinoma, oat cell" appear in this list and the indexing instructions to CARCINOMA, BRONCHO- GENIC are given with each. A few inversions appear awkward or linguistically uncomfortable, but they do provide an adit for a neophyte or hurrying indexer. Some neoplasm terms in the list are followed by "(unspecified)". This means that the term as used by the author in his text did not specify whether the neoplasm was benign or malignant. It may mean too that the author did not qualify the neoplasm by some de- scriptive adjective, such as "follicular", "juvenile" or others which may or may not be histologically mean- ingful. ii When a term is listed as (unspecified), whether qualified or unqualified, the indexer should follow the instructions given for indexing accordingly. However, when he sees (unspecified), he should look farther for possible qualifications which might - in many cases - result in indexing under an entirely different heading from the one marked as unspecified. For example, polyp (unspecified) is indexed as POLYPS and polyp, adenomatous is indexed as POLYPS, but polyp. fibrous is indexed not as POLYPS, but as FIBROMA. We borrowed a device from the 1951 Manual of the American Cancer Society, that of showing exceptions to histological typing. Angioblastic sarcoma is in- dexed as ANGIOSARCOMA except for genital angioblastic sarcoma. Following the form of the 1951 Manual we give two listings: angioblastic sarcoma (except of genitalia) which is indexed as ANGIOSARCOMA and angio- blastic sarcoma of the genitalia which is indexed as MESENCHYMOMA. Since this presentation is a manual of instructions for the indexing of histological types of tumors and cancers, most of the indexing instructions give headings for only histological types. But MEDLARS policy re- quires that each histological type be indexed also by site, with an organ/neoplasms pre-coordinated heading (MOUTH NEOPLASMS, EYE NEOPLASMS, UTERINE NEOPLASMS). This type of MeSH heading must be supplied by the in- dexer automatically and routinely. Obviously the organ site cannot be shown routinely in the instructions in this list. For a basal cell carcinoma of the forearm, the histological type is rep- resented in this list by entries reading "basal cell carcinoma" and "carcinoma, basal cell", both instruct- ing the indexer to index under CARCINOMA, BASAL CELL. But it is understood that in indexing, the indexer must also supply the required site headings - in this case, FOREARM and SKIN NEOPLASMS. In a few cases in this list, indexing instructions will give only an organ/ neoplasms heading instead of a histological type; in a few cases instructions will give both a histological iii type and an organ/neoplasms pre-coordinate to be helpful. INDEXING POLICY A few general words should be said here about the MEDLARS approach to the indexing of tumors and cancers. For the minute details of this area of indexing we have appended the section on the index- ing of neoplasms taken from the MEDLARS INDEXING MANUAL (Appendix V). Here, then, are some general principles in summary. As said above, every neoplasm is indexed in INDEX MEDICUS under both the site and the histological type. For example, a reticulosarcoma of the stomach is in- dexed under both STOMACH NEOPLASMS (the site) and SARCOMA, RETICULUM CELL (the histological type); ductal carcinoma is indexed under both BREAST NEO- PLASMS (the site) and CARCINOMA, DUCTAL (the hist- logical type). All cancers and tumors are indexed in both places even though a specific histological type is always localized exclusively in a given site. That is, al- though a ductal carcinoma - see the example above - is always located in the breast, both CARCINOMA, DUCTAL and BREAST NEOPLASMS are submitted by the indexer. It facilitates the retrieval operation in that a searcher does NOT have to request CARCINOMA, DUCTAL in order to make a complete search of BREAST NEOPLASMS, inasmuch as the indexer has routinely provided the site term. If a pre-coordinated term (BRAIN NEOPLASMS, IN- TESTINAL NEOPLASMS, etc.) is not available in MeSH for the specific site of the tumor, the anatomical heading in MeSH is indexed as an additional term for the site. For example, an article on a melanoma of the iris is indexed under MELANOMA (the histological type), IRIS (in the absence of a required concept iv "IRIS NEOPLASMS" - not in MeSH) and EYE NEOPLASMS (the pre-coordinated organ/neoplasms site term). Sometimes a concept is present in the name of a tumor which exists as a MeSH heading. For ex- ample, "telangiectatic sarcoma" is to be indexed, according to instructions in this list, as simply ANGIOSARCOMA. Apparently the idea of telangiecta- sis is inherent in angiosarcoma and the expression "telangiectatic sarcoma" is merely a synonym for the current preferred term ANGIOSARCOMA. The in- dexer should not index under TELANGIECTASIS unless the author makes a particular point of this aspect of the cancer. This example should serve as general indexing policy when the indexer is tempted to am- plify with additional headings: do not add terms un- less the author is particularly concerned with the concept. MALIGNANCY The degree of malignancy of a cancer is not ex- pressed in indexing at present. As a result, the identity of the tissue as a tumor or a cancer, or the stage of the cancer, Is presented in MEDLARS only as a "neoplasm." This is sad and sometimes yields inexact information in retrieval. The user can depend only on the exact histological identification as chosen from among the terms in this list. Happily, in most cases, the histological identity gives the user of INDEX MEDICUS a clue to the malignancy, i.e., SARCOMA is malignant, CARCINOMA, BASAL CELL is malignant, ADENOMA is not. METASTASES MEDLARS at present, moreover, does not identify the primary or secondary site of a tumor. A metasta- sis is indexed, as above, under the site term in MeSH v and under the histological term from this manual, with the additional MeSH heading, NEOPLASM METAS- TASIS. Consonant with the usual indexing policy of designating certain terms for publication in INDEX MEDICUS as IM (INDEX MEDICUS terms) and cer- tain other terms only for storage in the computer as NIM (non-INDEX MEDICUS terms), indexers have been instructed to print in INDEX MEDICUS both the site and histological terms, but to store within the computer the metastatic concept. Although the term NEOPLASM METASTASIS is NIM, both the site terms - the primary tumor and the secondary site - are indexed as IM. An article on uterine tumors meta- static to the lung is indexed as UTERINE NEOPLASMS (IM) LUNG NEOPLASMS (IM) NEOPLASM METASTASIS (NIM) but, unfortunately, an article on lung neoplasms metastatic to the uterus is indexed identically as UTERINE NEOPLASMS (IM) LUNG NEOPLASMS (IM) NEOPLASM METASTASIS (NIM) Let us hope that the future bring forth a more specific way of indexing the direction of the metastasis. CARCINOMA We warn the user about the term "carcinoma" as met in the literature. This term Is used frequently in all languages as a synonym for "cancer" and, as such, should not be confused with CARCINOMA as used in MEDLARS. "Carcinoma" in MEDLARS is only the histo- logical type as directed in this manual: it is never used when it means - when used incorrectly and loosely by authors - simply "cancer." vi USE OF THIS MANUAL It should be understood by users of INDEX MED- ICUS and MEDLARS that indexers never index any tumor or cancer term found in an article without checking this manual routinely. And, before the publication of this work, the indexers routinely checked annotated copies of the American Cancer Society "Manual of Tumor Nomenclature & Coding." The annotations were made with the approval of MeSH and have been followed since the publication of the 1951 and 1968 editions. The manual in hand has changed nothing: it merely makes more accessible in a single tool all an indexer needs to know in in- dexing tumors for INDEX MEDICUS. The terminology of authors describing tumors may or may not be that of the American Cancer Society or of the National Library of Medicine. Hence, EVERY neoplasm in EVERY article is checked against this list, whether or not the term used by the author is in MeSH. For example, what is called a "uterine fibroma" by an author is indexed histologically as LEIOMYOMA according to this list. Although MeSH contains the term FIBROMA, histologically a "uterine fibroma" is not a "fibroma": it is a "leiomyoma" and to have in- dexed under FIBROMA without consulting this list would have been wrong. Although the indexer is expected to use this tool routinely and although he will follow the indexing policy on supplying both histological and site head- ings for total indexing, he should remember that nothing in this manual is to interfere with basic policy regarding IM (INDEX MEDICUS) and NIM (non- INDEX MEDICUS or computer-stored) headings. vii APPENDICES The reader will find in this manual six ap- pendices to the basic neoplasm word-list which is really merely a conversion table for terms for tumors and cancers found in the literature to be indexed in INDEX MEDICUS and its derivative pub- lications . Appendix I is a list of pre-coordinated site terms in MeSH. Appendix II is an alphabetical list of histological terms for neoplasms found in MeSH. Appendix III is an additional set of MeSH neoplasm headings which indexers find extremely useful in qualifying the pathology and physiopath- ology of cancer, and useful in coordinating for greater specificity in indexing and searching. Ap- pendix IV is meant for the use of MEDLARS search analysts: it is a list of terms which are not ob- viously neoplastic but which are, nevertheless, indexed under tumor headings. V-l INDEXING PRINCIPLES FOR SUBCATEGORY C2 (NEOPLASMS, CYSTS, AND POLYPS) 17.1 Distinguish among neoplasms (tumor, cancer), cysts and granulomas: do not use pre-coordinated neoplasm headings for non-neoplastic cysts and granulomas. Index a non-neoplastic parathyroid cyst under PARATHYROID DISEASES (IM) and CYSTS (IM) and NOT under PARATHYROID NEOPLASMS. Likewise, index a non-neoplastic granuloma of the pharynx under PHARYNGEAL DISEASES (IM) and GRANULOMA (IM) and NOT under PHA- RYNGEAL NEOPLASMS. Hydatid cyst is not indexed under CYSTS: it is a synonym for ECHINOCOCCOSIS. Hodgkinfs granuloma is not indexed under GRANULOMA:it is a synonym for HODGKIN'S DISEASE, which is neoplastic. Malignant granuloma of the nose (lethal midline facial granuloma) is indexed as GRANULOMA, LETHAL MIDLINE, which is non- neoplastic. 17#2 No histological heading for a neoplasm may be used unless it is first verified in the Manual of Tumor Nomenclature & Coding of the American Cancer Society. It will be referred to in this section and elsewhere as the Tumor Manual. Verification in the Tumor Manual is necessary for often the histological term used by the author is not that used by the Tumor Manual. Al- though the term used by the author may be a MeSH term, the Tumor Manual must be checked routinely for every tumor or cancer an Analyst indexes. 17.3 If "carcinoma" is used by the author as a synonym for "cancer", do not consider the main heading CARCINOMA, either for IM or NIM. Instead, use only the pre-coordin< ated neoplasm heading. If the carcinoma, however, is qualified (as CARCINOM* EPIDERMOID), it must be indexed V-2 Subcategory C2 - 2 as IM. If, also, the Tumor Manual directs you to CARCINOMA, index CARCINOMA as IM. Do not interpret the word "lesion" (used loosely by some authors) as "cancer" (see 16.1). 17.4 INDEX MEDICUS at present makes no distinction be- tween malignant cancers and benign tumors. Both tumors and cancers are included in the term "neo- plasms" and we do not differentiate between the non- malignant and malignant. Often the degree of malig- nancy is inherent in the specific histological type as found in the Tumor Manual. If the malignancy or benignity of a cancer or a tumor is the point of the article, index the neoplasm as explained below, with the Subheading *pathology. 17.5 Since the literature on tumors and cancers is so great, MeSH has provided the site of neoplasms in the form of pre-coordinated organ/neoplasm headings for most of the organs in the body: BREAST NEOPLASMS, | UTERINE NEOPLASMS, CERVIX NEOPLASMS, STOMACH NEOPLASMS, etc. There are over 80 of this type of pre-coordination. 17.6 Index all neoplasms in two places: under the site in the form of the organ/neoplasm pre-coordinate and under the histological type as found in the Tumor Manual. An article on epidermoid carcinoma of the cervix goes under the site as CERVIX NEOPLASMS (IM) and under the histology as CARCINOMA, EPI- DERMOID (IM). An article on the surgery of epi- dermoid carcinoma of the cervix goes under CERVIX NEOPLASMS *surg- ery (IM) and CARCINOMA, EPIDERMOID *surgery (IM). 17#7 Index under the most specific pre-coordinated organ/ neoplasm term in MeSH (IM) and the histological type (IM).
