Expands the knowledge base related to the state of health information exchange (HIE) to support care for persons receiving long-term and post-acute care (LTPAC). The main tasks of the project included a targeted literature review, environmental scan, and key informant interviews to examine what is known about HIE to support transitions in care and shared care for persons receiving LTPAC.
