Abstract:
This report is intended to inform clinicians and health care managers about telephone care services and programs for patients with chronic illnesses. Specifically, the report addresses: (1) How telephone care services can contribute to improved patient care; (2) Characteristics of effective programs; (3) Patients most likely to benefit from telephone care; (4) Integrating telephone care services into systems of care; and (5) How to evaluate programs and identify areas for improvement.
